Request to Access and Consent for Release of Protected Health Information
If you need a listing of the prescriptions you have had filled in the past, the Health Insurance Portability and Accountability Act (HIPAA) requires you to fill out a form. To save time at the pharmacy, you can print out that form and fill it out ahead of time.

Morton Pharmacy accepts 99.9% of insurance plans. Below is a link to the prescription processors we accept. If you do not see your prescription processor, please bring your insurance card into any Morton Pharmacy and we will be happy to check and see if we accept your insurance plan. We do accept all Medicare Part D plans available in Wisconsin.
